Im thinking this has to do with your permissions. Im getting a forbidden error that has to do with permissions. make sure that directory is properly chmodded. Start with 777 to see if you can get it ...

The thumbnail's and other images are generated fine on my site. Im sure you just need to adjust your size's.

You might be uploading such a big image that it is blurred because it is compressed to such a small dimension.

I would try changing you category image width in your maps admin panel.

Make sure you have uploaded all the files in the correct directory.

Make sure you activate the module.
